[Post-traumatic hypoglossal nerve paresis due to occipital condyle fracture].

Acta Chir Orthop Traumatol Cech 76(4):335-7 (2009) PMID 19755060

The authors describe the case of a patient, a 33-year-old man, who presented with paresis of the hypoglossal nerve following an occipital condyle fracture. The patient, as a driver, was involved in a car accident and from the beginning he complained of neck pain. The diagnosis was established by CT examination as late as 72 hours after injury on the basis of clinical presentation (difficulty in swallowing, chewing and speaking due to impaired mobility of the left half of the tongue, with the left half showing a marked hypotrophy and muscle weakness). A conservative treatment was used - application of a rigid cervical collar for 3 months, which resulted in gradual neurological improvement.

We investigate the role of protease activated receptors 1 (PAR1) and PAR2 in mediating these effects. First we demonstrate Klk6 and the prototypical activator of PAR1, thrombin, as well as PAR1 and PAR2, are each elevated in murine experimental traumatic spinal cord injury (SCI) at acute or subacute...

SCI leads to TSP4 up-regulation in lumbar spinal cord that may play a critical role in mediating centrally mediated behavioural hypersensitivity. Blocking this pathway may be helpful in management of SCI-induced changes in nociception. © 2013 European Federation of International Ass...
